Am I entitled to it? My birthdate is February 27, 1939. What will I do in order to get my lifetime membership card from Philhealth? Thanks.

Database verification shows that you don’t have assigned PhilHealth Identification Number (PIN) yet.
Please be informed that under the Lifetime Member Program, members who are unemployed, have reached 60 years old and have at least 120 months PhilHealth premium contributions may enroll.
To register, simply submit the following directly to PhilHealth Local Health Insurance Office near your area the following documents:
Duly accomplished PhilHealth Membership Registration Form (PMRF)
Two (2) pieces of latest “1X1” ID picture
Photocopy of Birth Certificate or ANY two (2) valid IDs issued by a government official authority with birth date. Please present original copy.
In addition for SSS Retirees/Pensioners:
SSS Certification indicating date of effectivity of retirement/ pension and
Print out of monthly SSS Medicare contributions from January 1972 to June 1999.
After processing, the member will be issued his/her PhilHealth Lifetime Member Card, which shall serve as a valid ID to be used in all PhilHealth transactions.
